The Healthy Families Feeding and Nutrition Clinic helps parents develop skills to feed their baby, child, or themselves. One-on-one appointments can also support parents to make food choices that are best for their family. Our Public Health Nurses or Registered Dietitians can give support in person, online, or over the phone.

Order Vaccines
All vaccine orders can now be submitted electronically, by completing the online order form.
To submit an order via fax, please download and print the appropriate order form below. Completed order forms can then be faxed to 1-905-885-5352. Be sure to include a copy of your vaccine fridge temperature log.
- Routine Vaccine Order Form: To order regular publicly-funded vaccines and school-based vaccines
- High Risk Vaccine Order Form: To order publicly funded high-risk vaccines

Healthy Families Referrals and Resources
The HKPR District Health Unit offers a variety of FREE services to pregnant individuals and families with young children that can assist new parents and promote a healthy start for babies and children. Review our programs, referral forms, and resources included below.
Healthy Babies Healthy Children
The Healthy Babies Healthy Children (HBHC) Program is a free, confidential and voluntary program for pregnant women and families with children up to age six. The Program provides you with advice and support at various stages to help you give your children the best start in life.
The Nurse Family Partnership® (NFP®) Program is a free home visiting program where public health nurses visit young, first-time parents during their pregnancy and the first two years of their child’s life. NFP® can help parents: have a healthy pregnancy, have healthy children, and meet school and/or work goals.
Pediatric Nutrition Guidelines
The Pediatric Nutrition Guidelines (Birth to Six Years) for Health Professionals outline evidence-based nutrition and feeding guidelines along with red flags for healthy, full-term infants and children up to 6 years of age.
The guidelines were developed by members of the Ontario Dietitians in Public Health (ODPH). The document is posted at Pediatric Nutrition Guidelines
A Resource Guide for Engaging Clients Who Use Substances in the Perinatal Period
The Rourke Baby Record is a freely available knowledge translation/mobilization tool to enable efficient, current, comprehensive, and preventive health care by clinicians to children in the first five years of life. The 2024 version was launched in November 2024.
Well-baby/well-child visits using the RBR include growth and nutrition monitoring; developmental surveillance; physical examination parameters; immunizations; and anticipatory guidance on safety, family, relationships, and health promotion issues.
The RBR incorporates parent/caregiver resources such as age-specific information handouts and links to reliable parent resources on common topics.
