Immunization Records and Exemptions

Updating Records

The Health Unit does not receive automatic updates of immunization records from doctor’s offices. Instead, the Health Unit relies on parents and caregivers to keep their child’s immunization records updated.

Please note: COVID-19 immunization records are not accepted or required.

Parents/Guardians can update their child(ren)'s immunization record online using: Immunization Updates

 

Parents/Guardians can update their child(ren)'s immunization record by phone by calling the Health Unit at 1-866-888-4577 x 1507. You will need your child's name, date of birth, name of vaccine given and date the vaccine was given to complete the record.

Parents/Guardians can update their child(ren)'s immunization record by mailing the immunization record to a Health Unit office. The record will need to include your child's name, date of birth, name of vaccine given and date the vaccine was given.

Parents/Guardians can update their child(ren)'s immunization record by visiting a Health Unit office. The record will need to include your child's name, date of birth, name of vaccine given and date the vaccine was given.

Parents/Guardians can update their child(ren)'s immunization record by emailing* the immunization record to the Health Unit at immunization@hkpr.on.ca. The record will need to include your child's name, date of birth, name of vaccine given and date the vaccine was given.

*Email transmission cannot be guaranteed to be secure and it requires consent to be used to send personal and/or personal health information.

Immunization records that have been reported to public health are available online through Immunization Connect Ontario portal.

Your immunization record is not available online if:

  • Your immunization record has not been reported to public health.
  • Your health card number has not been provided to a health unit and/or entered into the immunization database.

If you can’t find your immunization record online, contact the office where you received your vaccinations and ask for a record of what they gave you. If you have provided the Health Unit with a record in the past and aren’t able to find it online, contact us for a copy.

Exemptions

Exemption for Religious or Conscience Reasons: 

If you are choosing not to immunize your school-aged child for religious or conscience reasons, you will need to: 

Watch the vaccine education video: 

Complete HKPR Vaccine Education Certificate Survey to receive a certificate.               

Complete a Statement of Conscience or Religious Belief Formand have it signed and stamped by a commissioner of oaths, notary public, or a justice of the peace. There may be a cost to have the form signed and photo ID is required. Or, check with the Service Ontario location near you, as they may do it free of charge.

Submit the certificate and completed Statement of Conscience or Religious Belief Form to Public Health one of the following ways: 

  • Online, click here 
  • Email: immunization@hkpr.on.ca 
  • Mail: HKPR District Health Unit, Vaccine Records, 200 Rose Glen Road, Port Hope, ON L1A 3V6 
  • In Person:  At anyHealth Unit office. Bring a printed copy of the certificate and completed form.

Keep a copy for your records. 

Exemption for Medical Reasons: 

If your child cannot be vaccinated for medical reasons, you will need to: 

Have your doctor or nurse practitioner complete and sign the Statement of Medical Exemption Form. 

Submit the completed Statement of Medical Exemption Form to Public Health on of the following ways: 

  • Online, click here   
  • Email: immunization@hkpr.on.ca 
  • Mail:  HKPR District Health Unit, Vaccine Records, 200 Rose Glen Road, Port Hope, ON L1A 3V6 
  • In Person:  At anyHealth Unit office. Bring a printed copy of the certificate and completed form). 

Keep a copy for your records.
